How Effective is CyberKnife for Prostate Cancer?

by | Aug 26, 2025 | Is CyberKnife Effective for Prostate Cancer

CyberKnife is a highly effective treatment for low- and intermediate-risk prostate cancer, with published studies reporting excellent long-term disease control and high survival rates.

As a form of stereotactic body radiation therapy (SBRT), it uses a precise, non-invasive, and condensed treatment schedule compared to conventional radiation therapy.

Effectiveness for localized prostate cancer
Clinical studies have repeatedly shown very high success rates for patients with localized disease, who are considered low to intermediate risk.

Low-risk patients: Five-year disease-free survival rates are reported to be between 97% and 100%.

Intermediate-risk patients: Five-year disease-free survival rates are between 88% and 97%.

Long-term control: Ten-year disease-free survival for low-risk patients is reported to be 93%, which is comparable to or better than historical data for conventional radiation.

Advantages over conventional radiation
CyberKnife, as a form of SBRT, offers several benefits over traditional radiation therapy for prostate cancer.

Fewer treatments: The entire treatment course is condensed into about five sessions over one to two weeks, compared to 40 to 45 sessions over nine weeks for conventional radiation.

Comparable or better outcomes: Recent studies comparing SBRT (CyberKnife) to conventional radiation have shown comparable or even slightly superior 5-year disease-control rates, with low side effects.

High precision and accuracy: The CyberKnife robotic arm delivers radiation with sub-millimeter precision from multiple angles.

It can also continuously track the prostate’s motion in real-time, allowing it to adjust the radiation beam and minimize exposure to nearby healthy tissue.

Less invasive: Unlike surgery or brachytherapy (radioactive seeds), CyberKnife is non-invasive and does not require incisions or implants.

Potential side effects
While the precise targeting of CyberKnife aims to reduce side effects, some irritation can occur because of the proximity of the prostate to the bladder and rectum.
Side effects are often temporary and minimal, but can include:
Urinary irritation (urgency, frequency, burning sensation)

Rectal irritation (constipation, diarrhea, bleeding) These side effects in particular can be reduced greatly with the use of SpaceOar Hydrogel prior to treatment.

Other potential side effects: Erectile dysfunction (sexual function can be affected, similar to other radiation treatments)

Fatigue

Suitability for different cases
CyberKnife is an option for many patients, including those with recurrent cancer or who are poor candidates for surgery.

However, patients with more advanced-stage cancer or higher Gleason scores are more likely to experience a recurrence regardless of the treatment chosen.

It is essential to discuss all treatment options with a physician to determine the most appropriate course of action for your specific diagnosis, overall health, and preferences.
